Health Insurance Claim

Intensive English Program (IEP) students are required to purchase the University-sponsored health insurance while enrolled at USF.  All IEP students must purchase the health insurance prior to registration. 

IEP students who are able to prove that they have comparable coverage under a United States-domiciled health insurance plan may waive this requirement. Proof must be in the form of a statement of coverage and insurance card and a signed waiver form for the applicable period of enrollment.  This form should be submitted to Health Promotion Services (UC 5th floor).  Students should not purchase insurance from their home countries, as it will not meet the University's requirement.

Spouse/Child Coverage
Additional coverage for a spouse or child can be purchased on the enrollment form. 

Fees and Enrollment
At the time of enrollment, IEP students must purchase health insurance coverage for their intended stay at USF.  Coverage dates vary based on specific programs and semesters.  See IEP Director or Health Promotion Services for specifics.  Enrollment forms are included in all IEP acceptance packets.

Payment must be a credit card (Visa or Mastercard only), a personal check or a money order payable to “Aetna Student Health”   See enrollment form for details.

To obtain a money order: Many supermarkets, post offices and convenience stores offer money orders.  Students can take cash to one of these locations and request a “money order” for a nominal fee.  The on-campus USF Post Office is located in Hayes-Healey Lower Level and is open Monday – Friday from 10am – 3pm.

Checks should be paper clipped to the enrollment form and sent by mail.  Credit card information is entered on the enrollment form and may be sent by mail, fax or email to:
